Posterior Aortic Root Enlargement (Manouguian Technique) and 19 mm Edwards Intuity Elite Aortic Valve Implantation in a 58-Year-Old Petite Female with Very Small Aortic Annulus

Posterior aortic root enlargement was described as early as 1979, and techniques of aortic valve replacement have evolved notably since then. A successful combination of balloon-expandable aortic valve implantation (Edwards Intuity Elite valve) and a posterior aortic root enlargement has not been reported to date. In the present case, a 58-year-old petite female was sent for cardiac surgery in a state of cardiac decompensation due to severe aortic stenosis and three-vessel coronary artery disease. Intraoperatively, a posterior root enlargement was combined

with the rapid deployment valve, reasons for which included the very small aortic annulus, a circular calcification of the aortic root, a complex procedure including four coronary artery bypasses, and the intraoperative need for shortening the operation by using a rapid deployment valve. Postoperative valve function and the clinical course were regular. It is proposed that the described procedure be employed for similar ‘bailout’ settings.
