Four patients with isolated cleft of the anterior mitral leaflet presenting with severe mitral insufficiency were operated on. In none of the patients was the cleft diagnosed preoperatively, and the diagnosis could only be established at surgery. Direct suturing of the cleft in all four patients, with additional annuloplasty in three, provided satisfactory results.
