A 10-year-old boy with severe aortic regurgitation underwent a Ross procedure employing the mini-root replacement technique. He had two eccentrically located coronary ostia, which were reimplanted as a single button into the pulmonary autograft. This report describes the technique of transfer of coronary ostia as a single button where coronary artery anatomy is unfavorable for individual transfer of these ostia.
