The combination of severe aortic and mitral stenosis is a serious condition that usually is very poorly tolerated from a hemodynamic standpoint and requires immediate treatment. The case is reported of a 44-year-old man affected by severe aortic stenosis and mitral annular calcification causing mitral stenosis, who underwent transcatheter aortic valve replacement (TAVR) and percutaneous balloon mitral valvuloplasty (PBMV).

A staged percutaneous approach, utilizing TAVR followed by PBMV, can be planned in selected patients with prohibitive surgical risk. In the present case the procedure was aimed at hemodynamic and clinical stabilization in order to add the patient, who was affected by severe kidney failure, to the transplant waiting list.
